Patient-Centered Medical Home (PCMH)

The basic unit of an integrated delivery system is the Patient-Centered Medical Home (PCMH). A PCMH facilitates the development of long-term, continuous healing relationships with a primary care provider and medical home team. It is the first place a patient calls when they have a non-emergent health concern and it is the coordinating entity for all of their care. A PCMH transformation is multi-faceted and requires a system-redesign and engagement of staff at all levels. Leveraging Dr. Temm’s research, we have guided practices transitioning to a PCMH through the following activities:

  • Empanelment of patients
  • Supporting electronic population health management system selection and implementation
  • Development of care management processes

The framework Temm & Associates, Inc. has developed serves to accelerate the journey to PCMH recognition. Through our expert training, coaching and resources, our client’s staff will be well-prepared to begin the transformation process. Under Dr. Temm’s leadership, we will assist in measuring a practice/group’s readiness for PCMH, share industry best practices, and provide the client with an array of resources to ease the process to accreditation.
