Patient-Centered Medical Home (PCMH)

The basic unit of an integrated delivery system is the Patient-Centered Medical Home (PCMH). A PCMH facilitates the development of long-term, continuous healing relationships with a primary care provider and medical home team. It is the first place a patient calls when they have a non-emergent health concern and it is the coordinating entity for all of their care. A PCMH transformation is multi-faceted and requires a system-redesign and engagement of staff at all levels. Leveraging Dr. Temm’s research, we have guided practices transitioning to a PCMH through the following activities:

  • Organizational leadership and change management through identification and mentoring of sponsors of change within the organization
  • Initiation of team-based care
  • Empanelment of patients
  • Supporting electronic population health management system selection and implementation
  • Development of care management processes
  • Development/refinement of patient access through a variety of system changes
  • Structuring risk assessment processes to identify and address patients’ gaps in care
  • Integrating behavioral health into the model

As an NCQA PCMH Certified Content Expert, Dr. Mary Temm partners with our client to develop efficient, reliable care practices designed for sustainable practice transformation. The framework Temm & Associates, Inc. has developed serves to accelerate the journey to PCMH recognition. Through our expert training, coaching and resources, our client’s staff will be well-prepared to begin the transformation process. Under Dr. Temm’s leadership, we will assist in measuring a practice/group’s readiness for PCMH, share industry best practices, and provide the client with an array of resources to ease the process to accreditation.
